Direct Primary Care Membership

Our patients enjoy unparalleled access to their doctor or nurse practitioner for a low monthly membership price. Membership includes unlimited office visits with extended times usually 30-60 minutes, annual physical exams, well-woman exams, routine cancer screenings such as pap smears, full skin exams, telehealth visits, and secure HIPAA compliant messaging. We never charge co-pays for visits or send unexpected bills. Certain additional services like procedures are available for a nominal fee.

Monthly Membership Pricing

Ages 0-18               $65/month 

(Requirement of 1 parent enrolled, otherwise $105/child)
Adults 19 - 39       $105/month

Adults 40- 64        $115/month

Adults 65+             $135/month

One Time Enrollment Fee

There is a one-time, nonrefundable enrollment fee of $100/patient or $150/family. There is a re-enrollment fee of $150/patient that must be paid to reactivate an account that is allowed to lapse without payment for 45 days or that has been voluntarily terminated by the patient.

Snowbirds and Seasonal Packages

We recognize that many members of our community spend only part of their year in Western Maine and the Mount Washington Valley, so we are happy to offer seasonal or half year memberships.

No Show Fee

Please note that we charge $40 fee if patients do not show up for appointments. We appreciate life happens and you may need to reschedule. If you need to reschedule, please call ahead so we can accommodate other patients.

Additional Discounted Services

We offer an array of a la carte services and do charge a nominal fee to cover the cost of materials and supplies, but will not nickel-and-dime you. We promote and practice absolute price transparency. Our price list reflects exactly what you pay.

A la Carte Procedures

Cryotherapy of skin lesions - $20

Skin Lesion Removal (per lesion) - $30-50

Suture/Laceration Repair - $40

Incision and Drainage (I&D, abscess) - $40

Toenail removal - $50

Joint Injections - $75 + cost of medicine ($30 corticosteroid)

Trigger Point Injections & Dry Needling- $50

In-office Saline infusion - $75

In-office Lab Draw (Phlebotomy) - $20

Electrocardiogram - $50

In house labwork with Quest Diagnostics

We draw our own labs here through Quest Diagnostics for a fee of $20 in addition to the cost of tests with transparent pricing up front. Patients may pay for the lab tests directly through Quest either with cash or insurance. We can also send lab orders upon request to any outside lab in the region if you prefer.

Point of Care Testing

We offer point of care testing for a range of common issues for a $20 fee. Available point of care tests include dipstick urinalysis, Strep A, Flu A/B, COVID-19, urine pregnancy, urine drug screening, fecal occult blood testing.

Home Visits

Do you have a deadline you have to meet and can't leave the office? Do you a sick child, but three other children are clinging to you, and you can't bear the thought of bringing them all in?  If you need us, we'll do our best to be there for you. (subject to availability and mileage; extra $50 fee applies)
